Payroll & Benefits Forms
The forms should be returned to your supervisor to be faxed to the payroll department at (217) 524-6875.
Please fill out BOTH forms because they need to go to two different departments for processing. The Comptroller's office needs the forms with original signatures mailed to the address below with a voided check or deposit slip.

Deferred Compensation Program (for those selecting a pensionable Per Diem position)
Deferred Compensation is a very beneficial 457 retirement savings plan. It is similar to a 401(k) plan, but it is for government employees. Our office highly recommends that new employees do not opt out of this benefit. State of Illinois Deferred Compensation website has more detailed information.
Information about Automatic Enrollment. Public Act 101-0277 amended the Illinois Pension Code to require automatic enrollment of certain new hires into the Deferred Compensation Plan. Effective July 1, 2020, any new State of Illinois employee that is a member of the State Employees Retirement System (SERS) and has not contributed to a retirement plan prior to July 1, 2020, will be automatically enrolled in the Plan.
New employees that are subject to automatic enrollment will have 30 days from their start date to opt out of the Plan. If a new employee is automatically enrolled, they will have 90 days from their enrollment to request a refund of their contributions. Plan participants may revoke their contributions at any time by contacting T.Rowe Price at rps.troweprice.com or call 1-888-457-5770. Representatives are available during business days between 6 a.m. and 9 p.m. central time.

Other Employment Information and Documents
REQUIRED ONLINE REGISTRATIONS:
WebHR - This is our attendance tracking system. You will be sent a link to activate your account after your employment documents are finalized. It will be sent to the email address on your application unless you specify otherwise. Either you or your supervisor will enter your daily attendance here in order to report your time to payroll.
Court Transcript Fees (CTF) - you will submit court transcript fee requests through this online system. If you will not be submitting for transcript reimbursement, you may skip this registration. If you will be getting a work email address, and prefer to use that for this purpose, please wait until your email address is assigned. If you have questions about if you will receive a work email, ask your supervisor. For tutorial videos on how to register for and use CTF, please click here.
MyPayStubs - Payroll stubs are available online only. You must register for an account after receiving your first check by direct deposit. If you use the same email address as the CTF account above, after registering for both, they will share registration passwords so you will only need to remember one. NOTE: Please get approved and log in to the CTF system prior to trying to register for PayStubs. The two systems are related if you use the same email address for both.
ShareFile - Court reporters and court specialists who prepare transcripts are required to backup files at least once a month. Click on the ShareFile link for more information and information on how to register. If you will not be preparing transcripts, you may also skip this step.
